Waiver Acknowledgment Required
Before participating in Paddle for Port, all participants must review the Waiver and Release Form below.
Please read the waiver carefully. Acknowledgment of this waiver is required to participate in the event and confirms your understanding and acceptance of its terms and conditions. Participants under 18 must have a parent or legal guardian review and acknowledge the waiver on their behalf.
Paddle for Port Waiver and Release of Liability
In consideration of accepting this entry, I, the undersigned, intending to be legally bound, hereby, for myself, my heirs, my executors and administrators acknowledge & agree that I am voluntarily & knowingly assuming all risks and injuries which are inherent, or in which may arise, from my voluntary participation in this event. I further, knowingly & voluntarily waive and release any & all rights and claims for damages that I have or may ever have against the Community Chest of Pt Wash., the Town of North Hempstead, all event sponsors, officers, agents & employees of all of the above for all injuries or damages suffered by me arising from, or caused by, my participation in said event. I attest & verify that I am physically fit & that my physical condition has been verified by a licensed medical doctor. Further, I hereby grant permission to use & release said rights to any photographs, or any other record of this event for any purpose whatsoever & understand that I will receive no compensation in connection.
